BWDSB FUNCTIONAL ABILITIES FORM (FAF)
If you require a medical leave of absence on the advice of your doctor, you will need to have your doctor or medical practitioner complete the Bluewater Functional Abilities Form (FAF), which can be found below.

This is the same form that you may need to have your doctor or medical practitioner complete when you return from a medical leave of absence.
Your doctor does not need to provide a diagnosis, but should use the FAF to indicate abilities and limitations.
Please submit the fully-completed form to your manager or administrator.
You do not have to apply for a medical leave through the EIL (Employee Information & Leaves Form); this will be done on your behalf by HR after they receive your completed FAF.
